College Visitation Request Form

It is the policy of Southwest Covenant Schools to allow juniors and seniors two absences per year from school in order to visit colleges and universities to which they many apply in the year following graduation from high school.

In order to facilitate the planned absence, we ask that you complete this form at least two weeks in advance of the planned campus visitation. No absences will be granted in May or in conflict with school events.  No more than two students may attend the same campus on the same date unless attending a specific campus event.

Once your form has been submitted, please pick up or download a Verification Request Form before the visitation date.  The form must be signed by an official of the college or university and returned to the school office.
